Is there any possibility that you have more than one Galaxy server process running? The logs only show that an upload attempt was made, but no indication that a job ever ran. If you're getting all the way to a red dataset with output from the upload tool, there should be a lot more in the log.

No, I don't see anything like that in the log after I started the upload. Any ideas on how to see what might be going wrong there?
Could you provide the contents of the log from the time you click the execute button on the tool form until it turns red in the UI?
